Your First Visit
Bring a photo ID, your insurance card, a list of current medications, and any relevant imaging or records from prior providers.
New patient paperwork needs to be finished before you are seen. Call 940-260-7741 ahead of your visit and we will send your forms so you can complete them at home.
If you would rather fill them in at the office, please arrive 15 minutes early. Paperwork completed after your appointment time may shorten your visit.
What to Bring
- Photo ID and insurance card
- List of current medications and allergies
- Comfortable shoes you wear most often (helpful for gait assessment)
- Any prior X-rays, MRIs, or medical records
Insurance & Billing
We accept most major insurance plans. Because coverage varies, we recommend confirming your specific plan with our office before your visit by calling 940-260-7741.
In-office procedures are performed onsite, while larger surgical procedures are coordinated with nearby accredited surgery centers and hospitals.
